Account Change Form (232B)
Use this form to make major changes to your account (i.e., add/remove co-signers, change of address or legal name, etc.)

Debit Card/ATM Card Application (473KB)
You can use this form to apply for both the ATM Card or the SLCU Debit Card.

Direct Deposit (43KB)
Use this form to set up Direct Deposit of your payroll directly into your SLCU checking account.
